Care and Support Workers

This care service includes a personalized program of personal and household care tailored to each individual service user through a care plan. The responsibilities of the carer will involve aiding the service user with various tasks while also ensuring their dignity, privacy, and independence are honored to the best of their ability.

Care Coordinator

The position of a care coordinator is often considered the most crucial role in the management of each patient. The coordinator oversees the coordination of various specialists required to assist the patient and evaluates the quality of care provided.

Live in Carer

Live-in Care Workers are hired to provide care and support to clients around the clock based on the care plan provided for each individual. The clients have undergone a person-centered assessment to determine their needs, which then informs their care plans.

The main responsibility of the care worker is to fulfill the needs of their clients while upholding their privacy, dignity, and independence as specified in the care plan.

The responsibilities will involve not only helping with the personal care needs of the client but also their social well-being. This may involve tasks such as shopping, cooking, and light housekeeping. The type of care provided can range from companionship to palliative end-of-life care, with care worker pay rates adjusting accordingly. Care worker must follow the instructions of the Registered Manager and the individual care plan. The goal of 24-hour live-in care is to allow clients to stay in their homes with as much independence as possible, while performing tasks in a professional, discreet, and compassionate manner while respecting the clients’ dignity, privacy, property, and confidentiality.

Day to day duties

Due to the broad scope of the role, the responsibilities of a care coordinator can vary significantly. The first step in their work involves meeting with the patient, their kinship carers, and other family members to discuss their needs, the available services, and the assistance they require.

Based on this information, a care plan is created, and the coordinator is in charge of reaching out to other care units or organizations to guarantee the proper delivery of services.

The typical daily responsibilities usually consist of the following:
